Autism, Cerebral Paralysis, Left Hemi paresis, Aphasia, Dysarthria, Dyslalia
NS, a 5 year-old girl is brought by her grandparents for consultation and treatment in October 1998. She was born at 9 months of gestation, C-section, 3.800 gr., with the umbilical cord around her neck. She had severe anoxia and aspiration of meconial amniotic fluid. She was in an incubator for 10 days and discharged. Her pediatrician referred her to a specialized center, has the diagnosis of cerebral palsy with left hemi paresis, autism, delay in her psycho-motor development, and aphasia. She walked after 30 months with uncertainty and frequent falls. She stopped using diapers after she was 3 yo. She has problems with gross and fine movements. Problems in receptive and perceptive language. She learned single words after she was 3 yo like "papá, mamá, tata, eto (esto), Ana, lech, oto (otro), gau gau (perro), Brrr (auto), ala (allá, amos (vamos)", but easily forgets them. She didn't learn numbers and she only repeats vowels "a" and "u". When she was 3 yo began with physical therapy (PT) with few improvements, phonoaudiology (PA) for years until now which helped in her pronunciation but she cannot learn new words. At 3 yo she was taken to a day care center with normal kids her age. At present she walks with uncertainty and collides with furniture or objects of any size that are in her way. She has difficulty moving the left side of the body. She runs with her left elbow flexed or bent and drags her left lower limb that stays rigid. At times she is restless, has no interest in television or her surroundings. She doesn't play with toys neither with friends or relatives her age. She was taken to several health professionals, the report of the CAT scan dated August 1993 is: "Light signs of cortical-sub cortical atrophy with bilateral temporal dysgenesis". The report in the EEG dated May 1995 is: "Normal". The report of the MRI dated September 1997 is: "Discreet cortical-subcortical atrophy. Calculated 6 of magnitude Evans. Ethmoido-maxillary sinusitis. Adenoid growth". They were told she has Cerebral Paralysis with left hemi paresis, Autism and Aphasia.
On physical examination she looks like a healthy girl. She doesn't look to the person's face speaking to her, she is restless, she doesn't answer questions. She ignores things or toys that have lights, noises, bright colored toys, stuffed animals and balls. She shows no interest in anybody in the room. There is a decrease in the musculature of both lower extremities, hypotonic muscles and rigidity in the left side of the body. She sits down in one side of the chair. She walks without seeing where she steps and she collides with furniture or other objects in her way. She climbs stairs with her right foot only and holding on to the rail. She goes downstairs sliding on her buttocks.
